This one works for me in 1.21-pre2:

gamemode adventure @a[nbt={SelectedItem:{id:"minecraft:carved_pumpkin",components:{"minecraft:custom_name":'"\\"Maximal Warden Head Cover\\""'}}}]

Make sure you have all quotation marks exactly like this when copying.
