go into a nether portal, and scratch your ear because it's itchy. all the while you're still walking, and you blink, and fall in a lava sea. as you attempt to put your armour on to extend your life, you get stuck in your inventory and the buttons to close it don't work. you manage to exit your inventory after a while, but when you resurface, you see the "you died" message.

Take off equipment to repair something, then jump off a height having forgotten to re-equip…

You can't swim in lava. It's not a normal liquid.
