what about the kusaragi station urban legend in japan? you could just build a train station with lights, no ppl or anything but like signs n stuff to where you are and also the drum sound thingy dont know if possible though

How about a man-spider thingy that crawls on every surface and when you look at it directly, it paralyzes you with fear and slowly crawls towards you and there's a 50/50 chance of it attacking or just plain running away. When it does attack you, it should inflict a type of poison only cured by one item like a golden apple or special potion.
It's a good idea, but it might suit an adventure mod better than a horror mod.
