Medusa (lancer) is her true name and what it says on the servant card, and this lancer version is basically a younger version of Medusa (rider)

while the anime/singularity presents her mostly in her first ascension outfit (a black cloak with the hood up), I decided well really I just asked Kirigiri which version to skin to skin her third ascension outfit
